You can fill out the paperwork at your home or work. You will then bring the paperwork in to start the examination process.

What To Expect On Your First Visit
Going to the doctors office is always a nerve racking experience. We will do our best to make your first visit as reassuring as possible. On your first visit, you will undergo an interview to determine the cause of pain and familiarize us with you as a person and your condition. A short orthopedic evaluation will follow to determine the precise cause of injury and the specific condition. This exam may include palpation, range of motion, orthopedic testing, and, if necessary, x-rays. From there you will undergo your care for the day. Immediately following care, you will be debriefed on your visit for that day. Afterwards, our elite doctors will prescribe a care plan that best suits your needs.
Remember, pain is the last thing to show up, and the first thing to disappear. Same as when you should finish prescribed antibiotics even though you feel better, you should finish the care plan your doctor prescribes you in order to achieve and maintain stability in your affected area.
